The Intake Process

After completing the initial interest form (located below), I will follow up with a brief 15 minute phone consultation to help us determine if we are a good fit for what you are looking for as well as my availability. After completing the appropriate paperwork, we will schedule an initial 60 minute intake appointment. At the end of this appointment you have the option to decide if you want to move forward with working together.

Follow-Up Sessions

I meet with clients on a weekly basis at a standing appointment time, for 53-55 minute appointments. The first few sessions are dedicated to treatment planning, continuing to identify goals and how we are going to address. your needs. This is a collaborative process and helps give us a map of how we are going to spend our time and prioritize what you want to get out of treatment. Frequency of sessions may increase or reduce over time depending on treatment needs.

Fees & Insurance

My rates for therapy are:

Intake session (60 minutes) - $225

Follow up sessions (53-55 minutes) - $200

I am an in-network provider for PacificSource and Blue Cross Blue Shield. Clients are typically responsible for a copayment for each session and you may be required to first meet a deductible.

I am considered an out-of-network provider for all other insurance plans. Therapy costs might still be covered as an out-of-network benefit. Because coverage for out-of-network benefits varies, I encourage you to contact your insurance company about out-of-network benefits and ask the following questions:

  • Is Erin Fettes, Psy.D. of Higher Heart Psychology an in-network provider?

  • What is my copay for each session? Do I need to meet my deductible first?

  • What is my annual deductible and how much have I spent towards my deductible so far?

  • If Dr. Fettes is not in network with my insurance, what percent will you cover per session? What percentage is my responsibility?

    • The CPT codes for sessions are: 90791 (intake) and 90837 (follow up sessions)

  • Are there any limits on how many sessions I can have?

I can provide you with a document called a superbill (this is a receipt for our sessions with the billing code, cost of the session etc.) to submit to your insurance company for reimbursement. You are responsible for the full session fee at the time of service if using out-of-network benefits.

Additionally, you may choose to self-pay for therapy. There are several benefits to self-paying for therapy, including privacy. When insurance is used, the insurance company has access to the client's diagnosis and can request other healthcare information, which you might prefer to keep entirely private. Additional benefits can be choice of therapist, as well as avoiding mental health diagnosis that insurance companies often require for treatment (you may not meet criteria for a mental health diagnosis!).

I accept most major credit cards and HSA/ FSA cards. I use Stripe on the platform SimplePractice to charge all credit and debit cards.

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers are required to provide clients who do not have insurance or who are not using insurance an estimate of the bill for health care services.

My rates are listed above, and a written estimate can be requested that includes a description of services, session fees, and diagnostic codes.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services, within one business day of your service.

You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Cancellations

Please let me know as soon as possible if you are unable to make our scheduled session. I do have a 48-hour cancellation policy, where if you miss our scheduled session or cancel with fewer than 48 hours before the appointment, there is a $60.00 fee associated. Insurance does not cover fees related to missed/late cancelled appointments.

Telehealth (Online Sessions)

I offer telehealth sessions through a secure, HIPAA compliant platform called Simple Practice, where you will have your own account and can access our virtual session.
